Mental Health/Mental Retardation

Mental Retardation Services

Potter County Human Services wants to provide help for  their mentally retarded clients and families by providing services not only during the critical times in one's life, but they also want the client to know that they're always there. They want to assist clients in the least disruptive approach while allowing for timely reaction to a client's needs based on professional assessment and evaluation of the problem. By offering family support services, vocational training, and community living arrangements, Potter County Human Services aim to keep their clients in the best settings possible:  their homes and their communities.

Mental Health Services

Potter County Human Services wants to provide help with their mental health services not only during the critical times in one's life, but they also want the client to know that they're always there. They want to assist clients in the least disruptive approach while allowing for timely reaction to a client's needs based on professional assessment and evaluation of the problem.

         Intensive Case Management

Linking to appropriate mental health services   (outpatient counseling, partial hospitalization programming, inpatient treatment, family based services and wraparound services).
Assisting consumers to obtain and maintain the necessary supports to remain in the community   (housing, respite, education, employment, medical care, food, recreation/leisure activities).
Access to twenty-four hour intensive case management.
Treatment planning, with the direct participation of the consumer and designed to meet their specific needs.
Twenty-four hour mental health crisis intervention.

        Resource Coordination

Same services as Intensive Case Management but is less intense, less frequent contact

Used as a step into mental health case management or a step down from Intensive Case Management
